The health assessment and intake questionnaire made available through the MEDVi platform are designed to collect information that may be relevant to determining whether an individual may be suitable for a weight-management program.

Completing an assessment, creating an account, or making a payment to MEDVi does not, by itself, guarantee eligibility for treatment or the issuance of a prescription.

MEDVi operates as a technology and care-coordination platform. MEDVi is not a pharmacy and does not itself manufacture, stock, dispense, or independently prescribe prescription medicines.

Medical evaluation and prescribing decisions are made by appropriately qualified and registered medical practitioners available through the MEDVi platform. Each medical practitioner exercises independent clinical judgment and retains sole discretion regarding whether treatment is appropriate, whether additional information or investigations are required, which medicine should be prescribed, the appropriate dosage and duration of treatment, and whether treatment should subsequently be changed, paused, or discontinued.

Information supplied through MEDVi's health assessment may be used to identify individuals for whom GLP-1 treatment may be unsuitable or who require further medical evaluation. Passing an initial assessment or screening does not mean that a prescription will necessarily be issued.

GLP-1 Treatment

GLP-1 medicines are prescription medicines and may not be suitable for every individual.

Any decision to prescribe a GLP-1 medicine is made by the treating medical practitioner after considering the patient's medical history, health information, investigations where appropriate, contraindications, potential benefits, and potential risks.

A practitioner may decline to prescribe medication, request laboratory tests or additional medical information, recommend another treatment, modify a patient's dosage, or discontinue treatment where medically appropriate.

Patients should use prescription medication only as directed by their treating medical practitioner and should inform their practitioner of side effects, changes in health, other medicines being taken, pregnancy or planned pregnancy, or any other information that may affect treatment.

Pharmacy Platform

MEDVi provides technology through which appropriately licensed pharmacies may participate on the platform and fulfil prescriptions issued to eligible patients.

MEDVi does not operate as the dispensing pharmacy. Medicines are dispensed and supplied by the licensed pharmacy fulfilling the patient's prescription.

Pharmacies participating on MEDVi are independently responsible for maintaining the licences, registrations, permissions, facilities, personnel, and other requirements applicable to their pharmacy operations.

The dispensing pharmacy is independently responsible for matters including the lawful procurement, storage, handling, dispensing, packaging, record-keeping, and supply of medicines fulfilled by it.

The pharmacy that fulfils a particular prescription may vary depending on factors such as the patient's location, prescription, medicine availability, inventory, pharmacy serviceability, and other operational considerations.

MEDVi may facilitate the transmission of prescriptions, order information, payments, delivery information, and other information necessary to coordinate fulfilment between patients and participating pharmacies.

Participation by a pharmacy on the MEDVi platform does not mean that MEDVi owns, operates, or controls that pharmacy or interferes with the pharmacist's professional responsibilities.

Where required, patients may therefore enter into a separate relationship with the pharmacy responsible for dispensing and supplying their medicine.

Medication Availability and Fulfilment

The availability of a medicine displayed or discussed through MEDVi does not guarantee that the medicine will be prescribed or available for fulfilment.

Medication availability may depend on the treating practitioner's prescription, pharmacy inventory, patient location, applicable requirements, and other factors.

The manufacturer, packaging, presentation, batch, or appearance of medication supplied by a pharmacy may differ from photographs or illustrations appearing on MEDVi, provided that the medicine dispensed corresponds with the applicable prescription and pharmacy fulfilment requirements.

MEDVi does not manufacture pharmaceutical products.

Weight-Loss Results

Individual weight-loss results vary.

Any weight-loss figures, measurements, testimonials, photographs, customer experiences, or other results presented through MEDVi are provided for informational purposes and should not be interpreted as a promise or guarantee of any particular outcome.

Results may be affected by numerous factors, including starting weight, individual biology, medical conditions, dosage, duration of treatment, adherence to treatment, nutrition, physical activity, sleep, lifestyle, and the recommendations of the treating medical practitioner.

Where MEDVi publishes aggregated or average customer outcomes, those results describe the relevant group or population measured and do not guarantee that an individual patient will achieve the same results.

Testimonials describe the experiences of particular individuals and may not represent the experience of every MEDVi user.

Healthcare Professionals

Doctors and other healthcare professionals providing services through MEDVi exercise independent professional judgment.

MEDVi does not direct a medical practitioner to prescribe a particular medicine and does not guarantee that any patient will receive a prescription.

A patient's participation in a MEDVi program, completion of an assessment, or payment of program fees does not override the independent judgment of the treating medical practitioner.

Medical practitioners may determine that treatment is inappropriate or that additional evaluation, laboratory testing, monitoring, consultation, or follow-up is required.

Laboratory and Other Healthcare Services

MEDVi may facilitate access to diagnostic laboratories, sample collection services, healthcare professionals, pharmacies, delivery providers, and other third-party services as part of the patient's treatment journey.

Unless expressly stated otherwise, these services are provided by independent third parties.

MEDVi may provide technology and care-coordination services connecting these participants but does not interfere with the independent professional judgment of doctors, pharmacists, diagnostic laboratories, or other regulated healthcare providers.

Program Fees and Payments

Fees paid through MEDVi may include charges associated with technology, program access, care coordination, consultations, diagnostics, medication fulfilment, delivery, or other services forming part of the patient's program, depending on the plan selected.

Where a medicine or healthcare service is supplied by an independent pharmacy, doctor, laboratory, or other provider, that provider remains responsible for the professional service or product supplied by it.

The way in which MEDVi collects or facilitates payment does not change the respective responsibilities of the medical practitioner, dispensing pharmacy, laboratory, or other healthcare provider involved in delivering the patient's care.

Information available through the MEDVi website, application, advertisements, educational materials, or other communications is provided for general informational purposes and should not be treated as a substitute for personalised medical advice, diagnosis, or treatment.

Accordingly, use of MEDVi may result in separate relationships between you and one or more such healthcare providers.

MEDVi does not hold itself out as the dispensing pharmacy solely because a pharmacy is accessible through, participates on, or fulfils prescriptions through the MEDVi platform.

Prescription decisions remain the responsibility of the treating registered medical practitioner, while the dispensing and supply of prescription medicines remain the responsibility of the licensed pharmacy fulfilling the applicable prescription.
